Scheduling
Surgery is scheduled on Monday, Tuesday, and Thursday. You will be given an opportunity to review the schedule with the patient coordinator after your consultation with Dr. Lawton. We will make every attempt to work with you to find the date that fits best into your schedule.
A $200.00 deposit is required to schedule your surgery. This payment is non-refundable and covers the administrative costs of scheduling. It is, however, considered a part of your overall fee and is not an additional expense. The $200.00 deposit is good for six (6) months from the day it is paid.
The balance of your total surgery fee is due two (2) weeks prior to surgery.
Surgery balance is payable by cash, cashiers checks or credit card only. We do not accept personal checks or credit card checks. |
If you should cancel your surgery after full payment has been made:
1 week prior to surgery |
you forfeit the $200 deposit PLUS 25% of the total surgery fee |
Within 72 hours of surgery |
you forfeit the $200 deposit PLUS 50% of the total surgery fee |
Within 24 hours of surgery |
you forfeit the full amount |
The fees for rescheduling your surgery are as follows:
1 week prior |
10% of the total surgery fee |
72 hours prior |
20% of the total surgery fee |
This agreement does not obligate you to surgery. This is just the pricing and cancellation policy for procedures at our practice.
